Clinical Pharmacist Practitioner – Oncology

 

Location - East Coast

​

JOB SUMMARY:

• Works as both chemotherapy preparation pharmacist and Clinical Pharmacist Practitioner (CPP) in the medical oncology clinic. Time will be divided evenly between the two job functions with the chemotherapy preparation in the morning and CPP duties in the afternoon.

• Reports to: Director of Outpatient Pharmacy Services

• Positions Directly Supervises Pharmacy Technicians, Pharmacy Residents, Students

 

CANDIDATES MUST HAVE:

• Completion of accredited PharmD program, licensed by the NCBOP in good standing, and completion of BCOP certification required. CPP accreditation required within 1 year of job acceptance. Completion of PGY1 residency program preferred.

• CCP=Clinical Pharmacist Practitioner

 

MAJOR JOB FUNCTIONS:

The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below, and specific duties may change from time to time.

1. Correctly enters chemotherapy orders in EPIC while verifying correct and appropriate dosages of all medications.

2. Checks for drug-drug, drug-disease, and drug-lab interactions for chemotherapy orders.

3. Correctly double checks chemotherapy orders placed by other oncology pharmacists.

4. Supervises oncology pharmacy technician specialists in the preparation of chemotherapy to ensure accuracy and safety of completed product before dispensing to patient.

5. Maintains accurate inventory levels by proactively looking at chemotherapy schedules to avoid delays in therapy while also decreasing inventory levels of products no longer being used.

6. Provides chemotherapy counseling as a CPP to patients before they receive their chemotherapy regimens and before any changes to chemotherapy regimens.

7. Provides chemotherapy patient office visits for specific reasons spelled out in the CPP agreement between the CPP and the practitioner (i.e.: nausea/vomiting).

8. Serves as pharmacy resource to providers in the medical oncology clinic.

9. Precepts pharmacy residents while on their medical oncology rotations in the Community Pharmacy and Ambulatory Care PGY1 tracks.

10. Serves as liaison between medical oncology clinic and specialty pharmacy by entering pharmacy orders to be sent to the specialty pharmacy, completing prior authorizations, and performing initial assessments for specialty pharmacy services.

QUALIFICATIONS:

• Education and Formal Training:

• Completion of accredited PharmD program, licensed by the NCBOP in good standing, and completion of BCOP certification required. CPP accreditation required within 1 year of job acceptance. Completion of PGY1 residency program preferred.

• Work Experience:

• One year pharmacist experience preferred. 

• Knowledge, Skills, and Abilities Required:

• Effective communicator and team player. Knowledge of medications including their mechanisms of action, proper dosages, and potential interactions. Knowledge of federal and state pharmacy laws. Proficiency in computer programs used for job functions. 

• Physical Requirements:

• Must be able to see and read physician orders and medication labels. Must have manual dexterity to type on computer keyboard and compound intravenous admixtures. Must be able to move about the chemotherapy compounding pharmacies and medical oncology clinics.
